Topiramate is an anticonvulsant medication, used to treat seizures and migraine headaches. Off-label, patients may buy topiramate for a range of other conditions, including other headaches (cluster, tension, etc), alcoholism, weight loss, PTSD, and several more.

This medication is known by a number of brand names, the most widely-recognized of which are Topamax, Topamax Sprinkles, Qudexy XR, and Trokendi XR. It is also possible to buy topiramate as a generic; patents have expired everywhere.

The medication can be used in patients as young as 2 years old in treating seizures, and as young as 12 for migraines. Other uses in children are highly variable and parents will need to seek dosage from a doctor.

Topiramate is known to be harmful to unborn babies if the medication is taken while pregnant. Note, however, that if taking the medication to prevent seizures, abruptly ending treatment can also be harmful. Topiramate for Migraines

Topiramate reduces the frequency of migraine headaches and, when they do occur, reduces their severity. Use in other types of headaches are less clear; studies are few and often produce contradictory results.

For migraine headaches in patients age 12 and up:

• The first week, start with 25 mg once per day.
• The second week, increase dosage to 50 mg per day, divided into two 25 mg doses.
• The third week, increase to 75 mg per day, divided into one 25 mg dose and one 50 mg dose.
• The fourth week, increase to 100 mg per day, divided into two 50 mg doses.

For cluster headaches:

• Start with 25 mg once per day.
• Titrate upwards at 25 mg/day every week until reaching 200 mg per day.

Treatment should continue for as long as it provides benefit and no adverse effects develop. For other types of headaches, such as tension headaches, dosage and duration of treatment is much less clear; patients will need to speak with a doctor.

Again, while patients buy topiramate to lower headache frequency, the medication will do little to alleviate a headache that does occur; patients can use other, more direct painkillers like NSAIDs or triptans to alleviate headaches in progress.

Eliminating Seizures with Topiramate

• Partial-onset seizures
• Primary generalized tonic-clonic seizures
• Seizures associated with Lennox-Gastaut syndrome

Topiramate can be used on its own or with other medications to accomplish seizure management. When used with other medications, or in children age 2 to 9, dosage can be variable as it depends on weight, other medications, and other personal variables.

In patients age 10 and up using just topiramate:

• The first week, start with 50 mg per day, divided into two 25 mg doses, taken morning and night.
• The second week, switch to 100 mg per day, divided into two 50 mg doses.
• The third week, switch to 150 mg per day, divided into two 75 mg doses.
• The fourth week, switch to 200 mg per day, divided into two 100 mg doses.
• The fifth week, switch to 300 mg per day, divided into two 150 mg doses.
• The sixth week, switch to 400 mg per day, divided into two 200 mg doses.

The majority of patients find 400 mg per day to be sufficient in managing seizures, though it can take up to two months for full benefit to be felt. Treatment may continue for as long as it continues to provide benefit. Treatment should not stop abruptly; speak with a doctor about a discontinuation plan.
